In this activity, students read a completed medical history form and answer seven multiple-choice questions about the information provided.

Preparation

Review the activity provided at the Example Web Site above.
Teach the vocabulary contained in the medical history form.
Put the link to this activity on the desktops of each student computer, or if you have your own Web site, link it there.

How-To

Review vocabulary related to a medical history form.
Direct students to the Example Web Site.
Have students read the medical history form and answer the comprehension questions.
Have the students show you their scores when they are finished with the activity.
Display the medical history form on the digital projector and review the answers with the class.

Teacher Tips

Students should have basic computer and Web page navigation skills before conducting this activity.

Make sure students know the words in the questions.Write the following four questions on the board:

1. Have you ever had surgery? What kind?

2. Hav you ever stayed overnight in a hospital? How long?

3. Have you ever had allergies? What kind?

4. Have you ever had high blood pressure? Do you take medication?

Have students ask at least 3 classmates the questions

Have classmates report back on their findings.

Today we are going to learn how to read a medical history form.

Project the from located at http://www.web-esl.com/health/MedicalHist.htm. Starting with the personal information,have students locate the Birth date, Birthplace, Employer, Insurance Company and Policy Number. Review the meaning of these words with the students.

Review how to ask questions and respond with answers to the personal information part of the form.

When was Roberto born?
Where was he born?
Is he married?
What company does he work for?

Continue practice all the questions for the personal information section.

Divide students into pairs. Have them practice asking and answering the questions.

Download and distribute this handout for the medical vocabulary.

Review the vocabulary and have students complete the worksheet.

Practice

Divide the students into pairs. have them fold their handout so that one student has the word and another has the definition. The student with the definitions, reads one and the other student says the word. Tell students not to go in order.
